What We Do

Community Solutions Limited provides the one-to-one assistance necessary for community re-entry and support in independent living for individuals who have sustained acquired brain injury or spinal cord injury. Community Solutions team is committed to facilitating and supporting the implementation of ongoing, community based rehabilitation and personal care. Rehabilitation Assistant and Community Support Worker services, programs and supports are directed and monitored by regulated health care professionals as well as supervised by our experienced Program Coordinators.

Rehabilitation and attendant care services are available 24 hours/day, 7 days/week in the person's own Home, School, Work and Leisure environments in both urban and rural areas across Ontario.

Community Solutions Limited endeavours to assist individuals in assuming responsibility for the widest possible range of skills and to live their lives as independently as possible.

Community Solutions Limited delivers services according to our Person/Community-Centered Service Model which:

  • Responds to an individual's life-long needs within a community context
  • Assists in a functional context, while emphasizing unique needs, strengths and abilities
  • Addresses life-long challenges and evolving needs/interests
  • Accommodates individual cognitive learning styles
  • Participates in developing and implementing strategies and/or supports that facilitate active community participation
